Dear Participant
Dao Lu CIC will be video-recording and taking photos during the Health Qigong practice
sessions in Walthamstow and Leyton. The films will be distributed to participants to
encourage self-practice at home. Some clips from the films may be shown from November
2021 on Dao Lu’s website and Dao Lu CIC’s social media channels as part of our continuing
promotion of Health Qigong and of Dao Lu. Some clips and stills may also be shared with
Sport England, William Morris Big Local, Waltham Forest Community Ward fund, funder of
these projects, as documentary evidence of how their grants were used.
We are asking your permission to use your image in the videos and/or photographs of the
sessions.
We mean no harm to the environment and we don’t profit from this project. The filming
conditions will remain professional: there will be no disturbances to the public or locations
and we only show the qualifying material.

Photograph & Video Release Form
I hereby grant permission to the rights of my image in photograph and/or video without
payment or any other consideration. I understand that my photographs and/or videos may
be edited, copied, exhibited, published or distributed and waive the right to inspect or
approve the finished product. Additionally, I waive any right to compensation arising or
related to the use of photos and/or videos.
Photographic and/or video recordings may be used for the following purposes:
- Launching events around the borough
- Educational presentations or courses
- Informational presentations
- Online presentation
- Social media
- Marketing material and fundraising
- Accountability reports, e.g. project report or annual report
By signing this release, I give my permission to display any photographs and/or video via the
Internet or in the public/culture setting.
I will be consulted about the use of the photographs and/or video recordings for any
purpose other than those listed above.
There is no time limit on the validity of this release nor is there any geographic limitation on
where these materials may be distributed.
This release applies to photographic and/or video collected as part of the sessions identified
on this document only.
By signing this release form, I acknowledge that I have completely read and fully understand
the above release and agree to be bound thereby. I hereby release any claims against Dao Lu
CIC utilizing this material for its purposes.
